BEFORE YOUR APPOINTMENT
One of our technicians will call about 14 days prior to your appointment to perform our pre-exam interview for medical history and additional details. Please be prepared to provide the name of your primary care physician, any specialists, medications taken, reason for the medications & a pharmacy of your choice. If this is not completed at least 72 hours prior to your visit, you will need to call to reschedule.
Our COVID exam protocol requires an Optomap retinal image to allow thorough evaluation while minimizing exposure. Insurance & medical guidelines specify we check your complete eye health during the routine eye exam. The $29 cost of the Optomap scan is not covered by most insurance. You will receive a text to pay for this amount and may pay this by either the provided link, check, or over the phone to our office no later than 1 week prior to your visit.
This fee is due regardless of coverage & is refundable after your eye exam if appropriate.
WHAT TO BRING
Please bring any recent or current glasses you have (i.e. reading, sunglasses, bifocal, computer) and contact lens information (box tops or photo.)
Please bring & wear a clean mask. Particularly because we are a health care facility, it is required to wear an appropriate mask throughout the visit. Valved masks or gaiters are not acceptable. If you choose not to wear a mask, we can reschedule your appointment for a later date.
Confirm you have not travelled internationally or tested COVID positive within 14 days of your appointment and have not been in contact with anyone who has done so. If so, we can reschedule you.
To reduce risk and overcrowding, adults must enter alone (unless a medical reason exists.) Those under the age of 18 however must be accompanied by a parent or guardian.
On arrival to our parking lot, call our main number 770.979.3456 to let us know you are here. Additional screening questions, temperature check, and hand sanitizing will occur prior to entering.
